In a 100 % run the player must earn 100 % item completion percentages as displayed by the in-game map, in every area.  This means all upgrades, as well as all large items, both of which have icons prominently displayed on the map.

In a 100% run the player must earn 100% item completion percentages as displayed by the in-game map, in every area.  This means all upgrades, as well as all large items, both of which have icons prominently displayed on the map.  It also means all unlockables, even if you already unlocked them they re-appear on subsequent playthroughs.  They also aggravatingly provide the same "blah blah unlocked" message which prevents you from knowing which ones you got and which ones you didn't get on previous playthroughs.  The results screen after Meio Prime will show 100%.

In a 100% run the player must earn 100% item completion percentages as displayed by the in-game map, in every area. This means all upgrades, as well as all large items, both of which have icons prominently displayed on the map.
