The best setup seems to be using a mouse but setting the in-game DPI really low. On a 1600 DPI mouse I have set it to 400 DPI. The reason you set it so low is so you can move more quickly and generate stronger hits.
Additionally, if you set your Graphics slider all the way to zero, the game actually stops rendering things like fire, dizzy stars, the laughing face when you taunt, and other stuff. It seems that it actually doesn't do any fire calculations, so this can be useful because fire causes explosions and explosions causes ringing, which slows down level completion. Having low Graphics setting also makes it easier to keep track of what is happening on the screen.
